As a Licensed Professional Counselor, Jerrica Adams, MS, LPC is required by law to maintain the privacy of your protected health information (PHI) and to provide you with notice of our legal duties and privacy practices with respect to PHI.

In the course of providing therapy services, we may collect and maintain information including your name, contact information, date of birth, insurance information, diagnosis and treatment records, session notes, and billing information.

When you submit a message through our website contact form, we collect your name, email address, phone number (if provided), and the contents of your message. We strongly advise against including any sensitive health information in website contact forms, as these are not end-to-end encrypted. This data is processed through third-party services (EmailJS and/or Formspree) and transmitted to our email inbox.

Website Analytics

Our website may collect anonymous browsing data including pages visited, time on site, and geographic region. This data is aggregated and non-identifiable. No PHI is collected through website analytics.

All clinical records are maintained through SimplePractice, a HIPAA-compliant electronic health record (EHR) and virtual session platform. SimplePractice maintains a Business Associate Agreement (BAA) with our practice and uses industry-standard encryption to protect your data.

Virtual sessions are conducted through a HIPAA-compliant video platform. Sessions are never recorded without explicit written consent.

We will NOT share your health information with anyone without your written authorization, except in the following limited circumstances required by law:

If applicable, records related to substance use disorder treatment are protected by additional federal regulations (42 CFR Part 2) and may not be disclosed without your specific written consent, except in limited circumstances.

For clients who are minors (under 18), a parent or legal guardian must consent to treatment. However, in accordance with Oklahoma law, we maintain certain confidentiality protections for minor clients, particularly regarding sensitive health topics. We will discuss the specifics of confidentiality with both minors and their guardians at the start of treatment.
